Open it on your computer/tablet, fill in and attach it to an e-mail and send to minnoff@westernsd.mb.ca.蜜桃传媒app If you submit the registration forms online, feel free to send a copy or photo of the required documents listed below via e-mail to minnoff@westernsd.mb.ca or stop in at the school and we can copy it for you.
You can fill in the paperwork and print it to drop it off.
We require the following documents when registering your child at school either online or in person:
蜜桃传媒app媋 birth certificate or other legal document with legal name and birthdate
a copy of your child's permanent resident document or student/visitor visa document if you are new to Canada
proof of address (utility bill, driver's license)
